Midwest Transitional Housing, Inc. provides Home and Community-Based Services (HCBS) to support adults with chronic conditions, disabilities, or health challenges. These services are offered through Minnesota Health Care Programs (MHCP) and are intended to help individuals remain in their homes rather than enter institutional care. We assist with daily tasks such as bathing, dressing, medication reminders, and light household chores.
Services may include independent living skills training and coordination with medical or behavioral health providers. Each care plan is person-centered and developed in collaboration with the individual to promote safety, dignity, and long-term stability at home.
